As artificial intelligence tools become more deeply embedded in health system operations, some of the most significant challenges are no longer technical, but cultural. At Michigan Medicine, clinicians and educators are already navigating the practical tensions that come with introducing AI into frontline care and medical training. From automated documentation to decision support and faculty evaluations, AI now shapes both how physicians work and how they are trained, raising complex questions about safety, responsibility, and professional development.

Much of the current discussion around AI in healthcare focuses on potential. Michigan Medicine’s deployment of tools like DAX Copilot, an ambient scribe and documentation assistant developed by Nuance, moves the conversation into a more immediate phase. The system listens to patient-clinician conversations, generates summaries, and offers optional decision-support suggestions. While this streamlines documentation, it also demands greater vigilance from clinicians who must verify and correct what the system produces. In this environment, the role of the physician shifts from originator to reviewer, prompting questions about cognitive workload, accountability, and efficiency.

Dr. Laura Hopson, associate chair for education in the Department of Emergency Medicine, framed the challenge succinctly. Even with advanced transcription tools, the outputs are not final until a human provider validates them. The risk is not that AI will act autonomously, but that unchecked reliance on machine-generated summaries may introduce subtle but critical distortions. The presence of AI does not eliminate clinical responsibility. It also relocates it.

Documentation Is Just the Beginning

While AI-generated notes may be the most visible application, Michigan Medicine is exploring AI in other sensitive domains. Max Spadafore, a clinical assistant professor in the Department of Emergency Medicine, has been using AI to assess the quality of faculty evaluations of medical students. These tools score narrative feedback on specific metrics and will eventually be used to help faculty improve their evaluations. This represents a broader trend across academic medical centers, where AI is being introduced to optimize administrative functions that shape career progression, competency assessment, and educational quality.

The implications here are dual. On one hand, AI can standardize subjective processes that have long lacked clear benchmarks. On the other, it introduces a new layer of opacity. Faculty may be evaluated not only by their clinical teaching but by how their comments align with algorithmic norms. The tension between human nuance and machine scoring is not unique to academic medicine, but it is particularly consequential in training environments that aim to cultivate independent clinical reasoning.

Spadafore acknowledged this complexity, noting that medical students must learn to use AI systems while also developing diagnostic intuition and judgment. The task is not just to adapt to new tools but to maintain the integrity of human decision-making within them. This balancing act may soon become the defining challenge of modern medical education.

Education Gaps Reflect Institutional Inconsistency

The uneven exposure to AI systems across disciplines within the University of Michigan highlights another emerging issue. While medical students are being introduced to AI tools during their clinical rotations, nursing students report little to no engagement with the same technologies. This disconnect suggests that AI adoption is not being managed as a strategic institutional initiative, but rather as a department-specific experiment.

In the long term, this may pose risks to team-based care models. If certain clinicians are trained to work with AI and others are not, the burden of verification, documentation, and decision-making could shift unevenly across teams. Worse, divergent expectations around technology use may erode trust or create confusion at the point of care. Bridging this divide will require not just broader adoption, but clear institutional policies that define roles, permissions, and competencies for AI interaction.

The lack of structured AI education in nursing programs also raises equity concerns. Nurses play a critical role in documentation, monitoring, and escalation. If they are excluded from AI literacy initiatives, their ability to interpret or question algorithmic outputs could be compromised. This is particularly concerning in high-acuity settings where ambient systems generate real-time notes and insights that inform rapid decision-making.

Workforce Pressure Complicates the Adoption Curve

Michigan Medicine’s use of AI is also shaped by practical constraints. As in many health systems, staffing shortages persist across departments. Spadafore pointed to ongoing gaps in nursing, radiology, and support services as key drivers of AI adoption. In this context, AI is not simply a tool for enhancement, but a compensatory mechanism for systemic strain.

However, this utilitarian framing carries risk. When AI is implemented primarily as a stopgap, questions around oversight, evaluation, and long-term sustainability can fall to the margins. The push to fill labor gaps may accelerate deployment before foundational processes are in place to govern performance and equity. The result could be a patchwork of AI tools deployed without a unified strategy, creating variable standards of care and inconsistent clinician expectations.

This is particularly important as ambient AI systems become more proactive. Tools that once served purely administrative roles are beginning to influence clinical recommendations, prioritize diagnostic possibilities, and shape physician-patient interactions. Without institutional consensus on what constitutes appropriate use, healthcare systems may inadvertently introduce variability in clinical judgment, precisely the problem AI is often enlisted to solve.

The Quiet Redefinition of Professional Identity

As Michigan Medicine continues to build out its AI capabilities, the cultural impact on clinical roles may become more pronounced. Physicians, educators, and trainees are already experiencing a shift in how expertise is defined and exercised. The tools being adopted do not just assist with tasks. They influence perception, workflow, and evaluation.

These dynamics demand a new kind of fluency, not just in how AI systems function, but in how they shape clinical culture. For educators, the challenge is not merely to incorporate AI into training, but to help learners navigate the ethical and cognitive tensions it introduces. For institutions, the priority must be to ensure that AI integration reinforces rather than replaces human judgment, particularly in high-stakes or ambiguous scenarios.

Michigan Medicine is not alone in facing these questions, but its visible experimentation across clinical, administrative, and educational domains offers an early case study.
