Greenway Health announces new population health solution, Greenway Community
Greenway Health announced an expansion of its population health solutions. Greenway Community is an integrated family of analytic, risk stratification, care management and data exchange solutions that enable providers to make more-informed decisions and deliver more-efficient, effective care.

Greenway Community works in concert with Greenway PrimeSUITE, Intergy and SuccessEHS electronic health record-based platforms — as well as other EHRs, health information exchanges (HIEs), immunization registries, hospitals and other healthcare systems — to aggregate the information that providers need to participate in emerging value-based payment models. Three elements of successful population health management
Greenway Community encompasses three major solutions — Greenway Community Analytics, Greenway Community Manager and Greenway Exchange.
