Greenway Health announces new population health solution, Greenway Community

Greenway Health announced an expansion of its population health solutions. Greenway Community is an integrated family of analytic, risk stratification, care management and data exchange solutions that enable providers to make more-informed decisions and deliver more-efficient, effective care.

With an aging population and healthcare spending on the rise, the nation’s clinical and financial health is dependent on efficient management of the health of each individual and that of whole communities. Sharable, actionable information is key to coordinating care and implementing best practices, and Greenway Community offers comprehensive solutions for analyzing, managing and improving population health.

Greenway Community works in concert with Greenway PrimeSUITE, Intergy and SuccessEHS electronic health record-based platforms — as well as other EHRs, health information exchanges (HIEs), immunization registries, hospitals and other healthcare systems — to aggregate the information that providers need to participate in emerging value-based payment models. Greenway Community delivers insight into both financial and clinical performance at the enterprise, practice, provider and patient level. This enables these groups to discover and incorporate best practices while providing the necessary proof of outcomes to payers and consumers who are part of a quality-focused plan.
